[Reminder] Development And Update Of New Control Drug Prescription Form

To: All Healthcare Facilities - Dubai

In accordance with Ministerial DecisionNo. (680) regarding the exclusion of some articles from Resolution No. 888 forthe year 2016 about the rules and regulations for describing and dispensing Narcotic drugs, Controlled and Semi Controlled drugs, and to DHA external circular No (96) regarding exceptions to the Ministerial Resolution No. 888 of 2016, we would like to mention the following:

  1.     In the absence of refill of the CD prescribed, the Physician should cross off the Refill box.

  2.  Incase of one Refill the Physician should fill the box under “Refill One” with the amount in figures of CD prescribed through the Refill box.

  3.     Inthe case of Two Refill the Physician should fill the box under “Refill Two” with the amount in figures of CD prescribed through the Refill box, in addition to step No (2).

  4.      The responsible pharmacist must fill the number of quantity issued for the first time in figures under the “Dispended” box and sign immediately on the same date of dispensing in the Pharmacist signature box below.

     5.    The responsible pharmacist must fill in Refill (1) the number of quantity issued in figures in case of one Refill with the signature on the “Refill One” as indicated in the pharmacist's signature box below.

   6.    The responsible pharmacist must fill in Refill (2) the number of quantity issued in figures in case of Two Refill with the signature on the “Refill Two” as indicated in thepharmacist's signature box below.

All health facilities must adhere to this circular in accordance with the instructions in this regard.
