Cancel Facility License

This service allows healthcare facilities to request for license cancellations. After cancellation of the healthcare facility license, the facility will no longer be allowed to provide patient services.

Who Can Apply

    • Delegated Healthcare Facility Representatives

Prerequisites

    • Paying all professional facility fines, if any
    • Withdrawing open appeals, if any
    • Cancelling all professional licenses (except the medical director)

Required Documents

    • Undertaking letter (If requested)

Service Steps

    1. Access the Dubai Health Authority website - log in

    2. Select cancel the healthcare facility license service

    3. Fill and submit online application.

    4. Pay required fees and outstanding professional and facility fines (if applicable)

    5. The application reference number created.

    6. DHA review. (Applicant must fulfill requirement as per Policy for Health Information Assets Management)

    7. If approved, the facility license is cancelled.
