Who Can Apply
- Delegated Healthcare Facility Representatives
- Supporting documents of the appeal
To appeal violations, proceed to your dashboard and click on the Violations widget.
- Open the violation you wish to appeal and click on the Appeal button.
- Submit online application
- DHA review
- Outcome will be communicated and violations will be updated based on the appeal outcome
- Draft applications that are inactive for over 3 months will be cleared from the account. A 15 day reminder will be sent to the registered email.
- Appeal must be submitted within 30 days of violation issue date
- Appeal outcomes are final and a violation can only be appealed once
