Request Frozen Specimen Transfer
This service allows healthcare facilities with active license offering fertility services to request for transferring frozen specimens like oocyte, sperm, etc. from one facility to another outside or within the jurisdiction of DHA including facilities outside UAE

Who Can Apply
- Medical Director of the facility
Prerequisites
Required Documents
- Filled application form
- Copy of Emirates IDs/Passports for the patient & spouse
- Marriage certificate
- Consent form signed by the patient to transfer the frozen sample
- Recent Medical report and NOC letter from the Primary/Exporting Facility including details of the frozen specimen
- Virology report (HEP B, HEP C, and HIV & Covid-19 PCR if applicable).
- Copy of the receiving/Importing facility’s policy on ensuring quality and safe transfer of the specimen
- For cases where the facility is not within DHA jurisdiction, NOC letter from the regulatory authority with a copy of facility license

- DHA review of request form
- In case of approval, a temporary permit is issued and confirmed via email
