Prerequisites

Required Documents

    • Updated trade license after initial approval

Service Steps

    • Access the Dubai Health Authority website - log in
    • Select Request to Change healthcare Facility Name service.
    • Fill and submit online application.
    • Pay required fees.
    • The application reference number created.
    • DHA review
    • In case of approval, DHA issues initial approval
    • Applicant to update the trade license at commercial authority with new name.
    • Upload the updated trade license with the new name in Sheryan
    • Receive the service via Sheryan.
    • New Facility License with the updated name is issued.

    • Draft applications that are inactive for over 3 months will be cleared from the account. A 15 day reminder will be sent to the registered email.
