Activate Facility License

This service allows healthcare facilities to activate their previously issued inactive license in order to start operating. The facility can also request to add add-ons in this service, which includes: home healthcare, telehealth, 24 hour facility, pharmacy delivery, pharmacy drive-thru, clinical training service, mobile unit (depending on the facility category, different add-ons are permitted).

Who Can Apply

    • Delegated Healthcare Facility Representatives


    • Adding and activating licenses of professionals under the facility

Required Documents

    • Select a medical director from activated professionals and provide an NOC from the medical director on appointment
    • Accreditation certificate copy (if required)
    • Valid trade license
    • Mobile Unit plan (if mobile unit add-on is requested)
    • Clinical Training Service Form (if clinical training service add-on is requested)

Service Steps

    • Fill and submit online application
    • Pay required fees
    • Inspection team gets in touch with the Facility to coordinate Inspection date and time
    • DHA conducts inspection
    • Facilities who pass the final inspection are issued active facility licenses (in case the inspection result was fail, the applicant must reschedule a new inspection and re-pay required fees)

    • Draft applications that are inactive for over 3 months will be cleared from the account. A 15 day reminder will be sent to the registered email.

    • Facility needs to have a valid inactive facility license to be able to apply for this service
